Substantiating claims in a personal injury lawsuit, class action, or mass tort case often involves a review of medical records. This is essential in cases where the alleged harm was physical—for example, an injury or illness caused by some action (or inaction) on the part of the defendant. Even people who are deemed ineligible for the lawsuit may benefit from learning of their status sooner rather than later.

We electronically source and deliver your digital health records, such as prescription histories and medical billing and diagnosis codes, to the law firm or litigation service provider reviewing your claim.

Security is our highest priority

We handle your records securely and in accordance with Fair Credit Reporting Act (“FCRA”) and Health Insurance Portability and Accountability Act (“HIPAA”) privacy requirements. Your personal health records are gathered with your permission and only upon your authorization, as provided when you applied to join a class action, mass tort, or other legal action. Your information is retrieved in real time and securely delivered to the firm screening potential plaintiffs, as authorized. This data is only accessible for the purpose of determining your eligibility and substantiating your claim. We never resell your data.

Our Process

    You or your personal representative seek to file or join a lawsuit, class action, or mass tort as a plaintiff.

    You or your personal representative sign a HIPAA authorization as part of the application process.

    Based on the authorization you or your personal representative provided, the organization reviewing your claim asks us to gather your electronic records.

    We gather your records and deliver them to the organization reviewing your claim.

To request an electronic or paper copy of your report, please provide the following information (using one of the methods on the right).

  • Your first and last name
  • Your daytime contact number
  • Your mailing address
  • Your date of birth
  • The last four digits of your Social Security number
  • An email address if you want the report emailed to you
  • The law firm or litigation services provider that processed your eligibility or claim

 

Please know:

  • Milliman IntelliScript will only have information about you if you have authorized the release of your records and the organization that you authorized subsequently asked us to gather your records.
  • Milliman IntelliScript will only have a copy of your report if you have authorized the release of your records at some point within the past seven years.
  • Milliman IntelliScript does not make decisions as to a potential plaintiff’s eligibility to file or join a lawsuit.
  • If you are looking for information about status as a plaintiff or a decision made about your eligibility to join a lawsuit, please contact the law firm or litigation services provider that you originally authorized to seek your records.
  • Your privacy is important to us. Individuals 18 years of age and older can request their own report. Requests can also be made by a designated personal representative such as a parent, guardian, or a trusted person granted the authority of a power of attorney.
