This data is only accessible to your insurance provider for the purpose of processing your application or, in some cases, evaluating a claim.

Request a Report
To request a copy of your report, please provide the following information (using one of the methods on the right).
- Your first and last name
- Your daytime contact number
- Your mailing address
- Your date of birth
- The last four digits of your Social Security number
- An email address if you want the report emailed to you
- The insurance company that processed your application or claim
Please know:
- Milliman IntelliScript will only have information about you if you have authorized the release of your records to an insurance company and that company asked us to gather your records.
- Milliman IntelliScript will not have a copy of your report if you have not applied for insurance in the past seven years or authorized release of information in connection with a claim.
- Milliman IntelliScript does not make the decision to grant or deny insurance. Milliman IntelliScript is not an insurance company, and we don’t have any information about the insurance company’s decision.
- If you are looking for information about your insurance application or the decision made about your insurability or a claim, please contact your insurance company or agent.
- Your privacy is important to us. Individuals 18 years of age and older must request their own reports.
